For Appointments:

Current patients:

• Call the office for availability.
• There is a $25 fee for missed/no show appointments.
• Call the office or send a message via the patient portal 24 hours in advance to cancel or reschedule.
• Patient portal can be accessed to verify next appointments.
• If you don't have access call the office to have the link emailed to you.
• Password reset link can be sent to you.

Potential New Patients:

• Must have a referral
• Once the referral is received it will be processed in the order in which it was received.
• Scheduling is done once a week.
•  Cancellation list is available upon request.
• There is a $25 fee for missed/no show intake and therapy appointments. 
  • Call the office 24 hours in advance to cancel or reschedule.

For Testing:

• There are 3 appointments
  • The initial appointment, Testing & Results
  • Results are 3 weeks after testing
  • There is a $100 fee for missed/no show testing appointments
    • Call the office or send a message via the patient portal 24-48 hours in advance to cancel or reschedule.
